How do I claim?

We’re committed to ensure your claim is settled in the shortest possible time. Our process is quick, efficient and designed to make it simple to use. That is why, for In case of an emergency, please contact Zurich Assist on (+62) 21 3044 9777.

To do this, you (the insured) must report a claim and submit the supporting document, less than 5 calendar days (120 hours) after your loss or accident. This can be done by registering your claim on claim.zurichclick.co.id. Or you can download a claim form here or contact us and we will send one to you.

You must not take any action or decision without contacting Zurich first and getting our consent. The process to submit and assess your claim is very straight forward, there are;

  1. Download or request a claim form,
  2. Fill in the claim form,
  3. Attach all relevant documentation (described below, by claim type),
  4. Submit to Zurich by email, fax or mail.
Please make sure that you submit all specific documentation as listed below. Missing or incomplete claim documents will delay the payment of your claim. Depending on the type of claim, we will need different information from you. However, for all claims, you will need to complete a Claim Form a photocopy of your passport and a photocopy of all plane tickets. Claims will be paid to the named Insured shown in the policy except for the payment of a death benefit, will be paid to the Beneficiary.

Medical Daily Compensation Claims

Death Compensation Benefit and Death Visit Compensation Claims

  1. Original copy of plane tickets
  2. Original copy of hotel receipts
  3. Medical report from the treating physician
  4. Original Police Report (if related to the accident)

Loss of Personal Baggage Claims
  1. Original Property Irregularity Report (PIR) from the airline
  2. Evidence of replacement from the airlines (if baggage is lost while within the responsibility of the airline)

Baggage Delay Claims
  1. Original official statement from the airline or transportation company stating the date, time and period of delay
  2. Original Property Irregularity Report (PIR) from the airline

Personal Liability Claims

  2. All original letters of claim from Third Parties

Claims Credit Card Protection

Claims Lost Revenue

  2. Attendance records and Original letter of absence from the employer
Claim Lost Money
  1. Original Police Report made within 24 hours of the incident
  2. Evidence of withdrawal of money or bank statements indicating money lost
  3. Original confirmation of cancelled travelers checks from bank
Travel Delay Claims, Additional Fees, Additional Cost Travel Substitute

Claims Trip Cancellation, Travel Delay

  2. Copy of the family (if the claim relates to the immediate family)
  3. Medical report from the treating physician

  5. The return / refund received
Golf Equipment Protection Claims
  1. Photo damage of golf equipment
  2. Receipts for repairs / improvements of golf equipment damaged or lost
  3. Letter of confirmation from a professional golf clubs that have occurred hole-in-one
  4. Proof of purchase for a victory drink (if there is a hole-in-one)
